(Oct. 8, 1906, Frank Shoaf, American, stable boss, was instantly killed at Ocean No. 2 Mine by falling from the top of the tipple head frame.  Shoaf came into the engine house about three o'clock on the morning of Oct. 8, had his lamp prepared, etc.  After a short conversation with the hoisting engineer he went to the shaft for the purpose of descending.  As Shoaf was on his way to the shaft the engineer lowered the right hand cage to the ground landing and as the cage was approaching this landing Shoaf signaled two, indicating that he was on the cage for an instant at the landing.  Then in response to the signal he began to lower the cage.  When the cage had descended nearly midway into the shaft he heard a noise resembling something falling.  He stopped the engine and made a hurried wexamination of the machinery and its surroundings, and found nothing wrong he lowered the cage to the bottom.  Resuming his examination he found the body of Shoaf near the base of the head-frame and the rope of the left hand cage off the sheave wheel. There were no eye witness in this case, but from the evidence found on the body, etc., it is reasonable to suppose that Shoafr opened the gate, gave the signal, and stepped into the left hand compartment of the shaft.  Seeing his mistake he grasped the rope and was carried up to the sheave wheels, at which point he was thrown from the roipe and fell to the ground.)
