(Killed.  Killed instantly by a roof fall, in the South West No. 1 "A" Mine, Sept. 5, 1903.);
(He was employed drawing a rib in company with John Beran, who was present at the time.  Caplaxa was engaged cutting out the coal stump.  Evidently the roof had not been properly posted, as quite a small portion of it fell and caught him, with said results.)

Clamerous, Mike

(Killed.  Killed by a fall of roof, in the South West "A" Mine, Dec. 17, 1904.);
(Dec. 17, 1906, Mike Clamerous, Polish, miner, and John Santimire, American, company man, were instantly killed at South West "A" Mine by a fall ofroof, while making a rib fall.  Santimire had been sent to Clamerous' place for the purpose of seeing that he did the work in the safest manner possible.  Santimire had been selected for this purpose owing to his mature judgement and experience in pillar work.  From the position in which the bodies were found it seems that Santimire was holding the light, while Clamerous was using the ax.)

(Jan. 9, 1906, Jacob Dubinski, Polish, miner, was fatally injured at the South West "A" Mine by a fall of slate, while knocking down coal.  He had been warned not to enter the place where the accident occurred, as it was in a dangerous condition.  However, he disregarded the order and had only struck a few licks when the slate fell upon him.)

(Emil Eardley, cager, was fatally injured.  He had stepped on the front end of the trip, as it was being landed at the shaft bottom, to cut off the haulage rope.  His foot slipped off the bumper, and he fell in front of the moving trip.  He died about 3 hours later.)

(He was riding on the front of his trip of three wagons, when he was flagged to slow up.  The brake is on the front of the wagon coming out, and it is supposed his head was caught between the wagon and coal when he stopped to put on the brake.  The body dropped into man-hole. Width of entry 7 feet 7 inches. Space between wagon and rib 2 feet 5 1/2 inches on one side, 4 1/2 inches on narrow side where Lane was caught.)

(Killed.  He was killed by a fall of roof, in the South West No. 1 "A" Mine, Dec. 17, 1904.);
(Dec. 17, 1906, Mike Clamerous, Polish, miner, and John Santimire, American, company man, were instantly killed at South West "A" Mine by a fall ofroof, while making a rib fall.  Santimire had been sent to Clamerous' place for the purpose of seeing that he did the work in the safest manner possible.  Santimire had been selected for this purpose owing to his mature judgement and experience in pillar work.  From the position in which the bodies were found it seems that Santimire was holding the light, while Clamerous was using the ax.)

The latter had examined the roof and instructed Shultz to place some posts to support it. He replied that he would do so as soon as he had completed loading his car, at which he continued to work, and before he had finished loading the car the roof fell and caught him.)
